Federal regulators released a final report Tuesday detailing hundreds of safety violations at a West Virginia mine where 29 men died, including 12 that it said contributed to the "entirely preventable" 2010 blast. The report was released hours after the announcement of a record $210 million settlement by the mine's new owners.
