Report blames Minneapolis nursing home for woman's deathMINNEAPOLIS (AP) — A state investigation has concluded that a Minneapolis nursing home is at fault in the death of a woman who rolled down a stairwell in her wheelchair.
The Star Tribune reports it happened May 18, 2009 at Providence Place, which is appealing the ruling by the Minnesota Health Department.
The department reported the nursing home failed to change the resident's care plan after she had twice before tried to open the door to the stairwell where she died.
The report says the woman suffered from behavioral problems, anxiety and depression. She also had a history of wandering the facility and trying to open doors.
She was found on the landing of the concrete stairwell, strapped into her wheelchair. Attempts to resuscitate her failed.
She was not named in the report.
